To aid in the application process, we have outlined the following steps as your guide:

1. Complete our application online or download our application in pdf form

2. Complete a Skills Assessment for your specialty area

3. Print and sign a job description for your discipline, i.e. (RN, LVN, RCP, etc)

4. You must provide documented proof of the following:

•  Current professional licensure and / or certification
•  CPR (AHA Healthcare provider course)
•  Specialty certifications (ACLS, TNCC, ENPC, etc...)
•  *Negative TB or CXR (current within 1 yr.)
•  *Hepatitis B Immunization, or declination waiver
•  *Immunization record documentation
•  *Physician's statement (current within 1 yr.)
•  *Minimum of 2 peer/ professional references
•  Competencies in field of specialty
•  Competencies in safety, environmental & privacy practices
•  Complete forms: *W4, *I-9, direct deposit (optional)
